On a chemicals, gases, cement or paper site, the small events are the early warning: a weeping flange, an alarm that keeps being reset, a hose in the wrong place. Tandm makes them quick to report, in the worker’s language, and writes the confirmed record into the system you already run.
What gets missed
Process sites run around the clock, and much of the work during shutdowns and turnarounds is done by contractors who have no account on your systems and may not read the language your forms are written in.
A minor release dealt with on the spot is often never recorded. The detail matters later: which line, which valve, what was in it, who was nearby and what was done. When the report is written at the end of the shift, that detail is the first thing to go.
